Clinical Elective Information

- Proof of Immunizations and Vaccination
Required by the Occupational Safety and Health Office at NCKU Hospital. The timeline is accounted from the starting date of observership.
Provide the form signed by the physician, a photocopy of the vaccination records, and the immunization test result at least one month before the elective starts.
- Chest X-ray: within ONE year exam report.
- Hepatitis B: within THREE years, either vaccinated or a blood test report for antibody and antigen.
- Measles, Rubella: within FIVE years, either vaccinated or a blood test report for antibody.
- Varicella(Chicken pox): within THREE years, either vaccinated or a blood test report for antibody.
- List of Accommodations You may also find your preferred accommodation via online booking platforms.
- Survey: to be completed after the training, we value your reflections; it matters!
